Main Function:  The Director of Health Homes will lead the Health Homes staff and manage the reporting process for all Leads.  S/he will be responsible for developing and enhancing the operating system of the Health Homes department.   S/he will provide direct management oversight on data tracking and reporting for Health Home. S/he is responsible to assure that the department achieves the Health Home standards, and ensure compliance with CMS and in concert with the organization's policies and procedures.

Responsibilities:
Department Management
  • Administers day to day operations in order to meet goals and objectives of the Health Home department.  
  • Provides administrative support to the Director of Clinical Services, support staff and outreach team. Coaches and mentors supervised staff to improve overall departmental performances.   
  • Directly supervises Assistant Director of Health Homes and Outreach and Intake Supervisor. Evaluates HH staff work performance based on core competencies, goal and objectives for the department.
  • Monitors HH overall census in order to achieve financial goal and objectives. Attend quarterly variance meeting with Fiscal Director.   
  • Performs ongoing oversight of all HH database and interface with lead health homes to address any specific issues related with the lead’s database.
  • Creates and revises all policy and procedure manual and train all HH staff on all revisions of the manual.
  • Completes required monthly reports, quarterly reports and capacity sheets. 
Quality Assurance and Continuous Quality Improvement
  • Collaborates with the Director of Clinical Services to ensure that all HH staff meet the required deadline to enter documentation in all databases in a timely manner.
  • Continuously improves  Health Home operational/tracking systems, processes and policies in support of the organization’s mission specifically, support better management reporting, information flow internally and externally, and staff reporting and data entry effectiveness.
  • Collaborates with the Director of Clinical Services to recruit and hire qualified candidates for the HH Department.
  • Performs annual reviews on all policies and procedures including HH, HIT, program, CQI/QA.
  • Develops functional systems to improve client satisfaction on the delivery of quality HH services. 
Fiscal and Data Management
  • Provides direct management oversight on all care management technology systems.  .
  • Delivers all required reports to the Health Home leads.  
  • Collaborates with the Fiscal Director to develop annual budget and monitor monthly variances. 
  • Maintains constant communication with the Biller and improves infrastructure to minimize billing disruption.
Lead Health Home Partnerships and Business Development
  • Participates in external HH meetings and Health Information Technology committees.
  • Devises marketing strategy to improve community awareness about the organization HH services.
  • Enhances and develops partnership with Health Home leads, contract managers and MCOs. 
  • Represents DCS at public forums, events and community meetings, and with funders and government entities.
  • Analyzes DCS Health Homes in relation to development and implementation of HH policies and DSRIP changes by the state. Provides training to HH staff as new processes are implemented.
Organizational Leadership
  • Attends all internal management meeting including operations, CQI/QA committee and PMT meetings.
  • Works in partnership with leadership to develop and implement organizational strategies, policies and practices.
Other responsibilities as assigned.
Qualifications:
Master’s degree in Social Work (license required), Business Administration, Public Administration or in Public Health, with at least two years of managerial experience in a clinical setting.   Incumbent must have a solid experience in managing and motivating staff to increase efficiency.    Experience in providing services to culturally/racially diverse communities and families; excellent communication, organizational and time management skills; exceptional interpersonal skills   Language proficiency in Haitian Kreyol or Spanish a plus.  The incumbent is able to work collaboratively in a team setting.  Strong computer literacy skills in Microsoft Office, care management software, and online health home tracking and billing systems.
