Position Summary

 

Community Health Specialists connect/link patients that have barriers to care, support preventive aspects of health, assists patients in accessing psychosocial and/or health services and support patient empowerment through health education and coaching. Community Health Specialists develop and maintain collaborative relationships with community key stakeholders, gatekeepers and other community organizations, assist patients in navigating health care and social transitions, and support patient empowerment through health education and coaching.


Core Job Responsibilities

 

• Engage patient assigned or referred for community health specialist in a collaborative on-going relationship to help facilitate their care
• Promote programs and services for families and provide health care and social information that encourages self-management
• Market ACCESS programs and services and empower people to engage in the health programs
• Educate on basic medical illness, diseases and/or behavioral health needs.
• Assess patient’s strengths and needs and develop a plan for intervention. Care planning and coordination is done in collaboration with an interdisciplinary team
• Provide ongoing follow-up with patients and/or service providers to determine whether patients have accessed services. Follow-up should be continuous from initial identification through case closure.
• Make home visits and conduct case management activities in community settings as required by program guidelines
• Conduct or co-lead group interventions as required by program guidelines
• Serve as part of the patient’s care team and support health center operations and attainment of organizational metrics
• Establish effective and respectful relationships with patients, families, professionals, payers and other relevant parties
• Assist in developing/maintaining community referral relationships and effectively connecting patients and families to community resources
• Engage in community planning groups and/or meetings to support the social and health care needs
• Using information systems and decision support, maintain a risk-adjusted caseload, and provide direct case management services to address specific issues affecting their health risk or health status
• Complete documentation and data entry as needed to assure optimal patient care and program reporting – track outcomes of outreach efforts
• Participate in evaluating outcomes at the individual level with each patient and at the same time participate in agency-wide evaluative and quality improvement efforts
• Obtain & maintain certifications/licensure/trainings as needed
• Perform other duties assigned


Requirements/Preferences

 

  • High School diploma required; Bachelor’s degree preferred in a health-related field (social work, psychology, counseling, rehabilitation, gerontology, sociology, or other human service field).
  • Minimum one (1) year customer service experience or two (2) years of experience in a public or private social service program or health care setting required
  • One (1) year call center or case management experience or two (2) years of experience in a public or private social service program or health care setting preferred
  • Demonstrated knowledge of working in a community based or public health setting preferred.
  • Basic familiarity with medical terminology required
  • Intermediate proficiency with Microsoft Office products (specifically, Word, Excel, Outlook) preferred
  • Bilingual English/Spanish preferred

JOB SUMMARY:

This position provides care to Schaumburg residents requiring intermittent home visits.  Using the nursing process, the essential core of practice, the registered nurse delivers holistic, client focused care. This position requires the registered nurse to work collaboratively with other village employees and area agency staff in assisting residents in challenging situations.    This position assists with employee and community clinics following standing orders, which are signed by the Public Health Advisor.  Nursing services are based on the American Nurses Association Scope and Standards of Professional Nursing Practice. 

The ideal candidate will need to effectively communicate to a full range of people, be able to work cooperatively and effectively with others to achieve common goals, and shift gears comfortably. To succeed in this role applicants will need the capacity to understand and be sensitive to the feelings of others. 

JOB DUTIES:

  1. Acts as a consultant to residents, family members or guardians, employees, area agency staff regarding available services in the Schaumburg area.
  2. Collects and analyzes comprehensive data pertinent to the resident’s situation and identifies actual or potential health conditions or needs.  This may include the need for home assistance, medical equipment, connecting the resident to community resources or area agencies, and options for medication adherence.                           
  3. Coordinates care delivery by sending the physician the Client Status Information (CSI) form, if necessary, completing all verbal/written orders, updating the physician regarding changes in the resident’s condition, reporting adverse medication reactions, along with acting as an advocate.
  4. Promotes a healthy lifestyle and a safe environment by educating the resident regarding their medical condition, discussing disease prevention strategies, encouraging healthy behaviors, and steps to reduce the risks of a fall. 
  5. Offers screenings, immunization/wellness clinics, and educational programs to employees, residents, community groups, and independent living facilities.
  6. Contributes to an environment that is conducive to the clinical education of nursing students.    
  7. Participates in the interdisciplinary management of clients who live in a hazardous home site.
  8. Participates in emergency preparedness activities and assists with emergency sheltering or Point of Distribution (POD) operations, if needed.
  9. Dedicates self to the core values of customer service, teamwork, integrity, respect and trust with the goal of providing excellent services and programs to meet the needs of the community and those within the organization
  10. Performs other duties as assigned.

QUALIFICATIONS:

  1. Bachelor’s Degree in nursing from an accredited college or university.
  2. Registered Nurse licensed in the State of Illinois.  Completes 20 contact hours of education as required for license renewal.
  3. A minimum of two years of experience as a professional nurse.  Public health, home health, geriatric, or rehabilitation nursing experience preferred.
  4. Possession of a valid driver’s license.
  5. CPR certified.
  6. Proficient with current computer software systems including email, calendar programs, job specific software, and customer service systems preferred or within the first six months of employment.

Health Justice Intern – The Praxis Project (Temporary, Full Time) (Oakland)

Who: The Praxis Project is a values-driven, national nonprofit organization that seeks to improve health justice through partnerships to build community power. Praxis believes strongly that organized communities are critical partners in the struggle to create healthy communities. Praxis works with over 200 diverse grassroots groups across the United States that influence local policy, mobilize communities, and strengthen programs and organizations to improve health justice. Praxis works in strong partnership to enable local efforts to build community power through training, technical assistance, and capacity building services, as well as comprehensive fiscal sponsorship. Our capacity-building programming is focused on partnerships that actively advance Praxis’ mission to build healthy communities by transforming the power relationships and structures that affect our lives. 

What: internships are designed to provide interns with a very real experience of health justice policy, public health training and capacity building, and field organizing. For candidates with extensive training and experience, this will be an opportunity to apply your skills to national projects impacting various communities; for candidates with less direct experience and training, this is an opportunity to learn the current national landscape of community-based organizing, people-centered training methodologies and grassroots campaign strategy, while exploring the fields of public health and communications. Depending on experience and education, the Health Justice Intern (HJI) will report either directly to a Praxis Director or to a Program Manager. The HJI will:

Data Analyst - MAPSCorps (Chicago)

Who: MAPSCorps (Meaningful-Active-Productive Science in Service to Communities), a nonprofit organization that provides urban youth with meaningful employment and STEM training opportunities through an innovative technology-based community asset mapping program. MAPSCorps partners with NowPow, a healthcare technology company that consumes the MAPSCorps data to connect community residents to the vital information needed to stay well and live long. NowPow, a for-profit social enterprise, and MAPSCorps, a 501c3, together make up a self-sustaining collective social impact business. MAPSCorps and NowPow catalyze communities by building the workforce of the future and improving the health and vitality of people, businesses, and organizations.

What: The Data Analyst reports to the Executive Director. The Analyst works closely with the Program Manager and is responsible for data processing, reports, and data visualization. During the data collection (mapping) period, the Analyst will supervise seasonal Data Coordinators to ensure efficient data processing and high-quality data. The Data Analyst also manages the MAPSCorps data requests process and mines the MAPSCorps datasets to provide reports and data visualizations to management and partners. The Analyst will work with the Program Manager on outreach efforts to grow data partners and users. The Analysts will also work closely with the MAPSCorps’ engineer to maintain and optimize annually the database, data collection tools, and website. In addition, the Analyst will provide guidance and support efforts to train youth on basic data analytics and visualization techniques. Due to the start-up nature of the organization, there will also be times that will require the Analyst to contribute to other organizational tasks such as reviewing grants, and assisting with program implementation and reporting requirements. What: IPHI is seeking an energetic and well‐organized team member to provide high-level planning, administration and operations support for a national grant-making and community support program. IPHI operates Data Across Sectors for Health (DASH) which identifies barriers, opportunities, promising practices and indicators of progress for multi-sector collaborations that connect information systems and share data for community health improvement. This position supports our national work at the nexus of health data, community collaboration and the social determinants of health, including support to cutting-edge communities across the country. Duties and responsibilities include program management and administration, communications and networking support, grants management and community support, and meetings and events management. 


Qualifications: Demonstrated experience and credentials in public health, public policy, healthcare, community services, or a related field. Minimum of one year of work experience in project or grants management, events management, health informatics, budget and financial reporting, health data and information systems, or program performance and evaluation. Candidates can write well, work fast, and succeed independently and in teams. We are especially interested in candidates who have experience working with diverse populations in a variety of community settings. The purpose of this position is to manage all Healthy Start (HS) projects related to quality assurance, quality improvement, program performance monitoring, and evaluation in accordance with HRSA guidelines; this position is responsible for the creation and monitoring of systems that pertain to data collection, analysis and dissemination including professional publication that advances the field of maternal and child health and the factors that contribute to infant mortality including racial, ethnic and socioeconomic factors.

Position Qualifications:
Minimum Education and/or Experience:
Bachelor's degree in Public/Business Administration, Public Health or a closely related field and two (2) years of experience in conducting research studies, including compiling data and writing reports and summaries related to public program design, implementation and evaluation. A combination of related education and experience may be considered in substitution for the minimum experience qualifications on a year per year basis.

Specialized Training, Certifications, and/or Other Special Requirements:
Degrees/credits must be from an academically accredited college or university as recognized by the U.S. Department of Education (USDE) or the Council for Higher Education (CHEA).

Knowledge, Skills, and Abilities:
Must have a general understanding of relational database design as it relates to data gathering and reporting applications; Must be highly detail oriented; must possess a working knowledge of data collection and evaluation principles and strategies and possess the ability to effectively communicate complex ideas, data and reports. Knowledge of maternal and child health indicators and the factors that contribute to infant mortality. Knowledge of the racial, ethnic and socioeconomic diversity of the population, well as health care and social service resources available in the area. Preferred Education and/or Experience:
Master's degree in Public Health; Experience in program design, implementation and evaluation preferred. Advanced skills with Word, Excel and Access.

Participate in emergency response training and respond to incidents as necessary.

Essential Job Tasks:
Design and monitor data information systems, including data collection and reporting, in compliance with all grant requirements in order to obtain the following information: participant demographics, health and psychosocial history, utilization of community resources, assessment results, participant specific program interventions, and measurable participant outcomes in relation to program interventions. Responsible for ongoing quality assurance which includes a comprehensive analysis of data systems on weekly, monthly and annual basis in order to identify staff compliance and accuracy of data entry; implements Continuous Quality Improvement (CQI) projects related to the data information systems in order to enhance program service provision. Prepare comprehensive monthly reports for Program Supervisor detailing data analysis findings in relation to program performance. In compliance with grant requirements, develop and implement ongoing data and performance evaluation to measure the impact of Healthy Start services in improving women's health before, during and after pregnancy. Manage sub-contract for external evaluation consultant as needed, including the scope of work and monitoring of contract deliverables. Evaluation plans must include an assessment of how local Healthy Start services increase community capacity to improve perinatal outcomes and will include partnerships with county and state epidemiology personnel, community data partners such as Maricopa Integrated Health Systems (MIHS), the Office of Family Health (OFH) Information Management Analyst and project staff. Responsible for timely query and submission of all data related reports, such as Managing For Results, First Things First, Navigator and HRSA data submissions. Work closely with Maricopa County Department of Public Health (MCDPH) Epidemiology Department to complete research and documentation for related professional publications and public health conference poster presentations. Works closely with Healthy Start program leadership to ensure seamless implementation of Healthy Start services across the Program; attends regular Healthy Start case conferences and team meetings; provides staff training and ongoing technical support regarding the data information systems.
