The Community Health Outreach Worker provides health education, disease management education, health care and social service navigation services, and/or case management services to patients/community residents. The Community Health Outreach Worker will work within the community to establish relationships and maintain a presence amongst the community and community partners.

ESSENTIAL JOB FUNCTIONS:

  • Provide pertinent health education and health behavior information to patient population/priority communities with the intent of assisting community members in improving health.
  • Establish positive learning expectations for patients/participants and their families by assessing their needs and explaining information in a clear and concise manner.
  • Work closely with patients and/or families to provide health education in various settings such as during home visits, health fairs, community events, clinic, hospital, ER, or via phone.
  • Under supervision, assist patients/participants/community members in obtaining the appropriate needed health services (e.g., doctors, follow-up exams, medications, referrals, etc.) and/or social services.
  • Follow-up on all issues until resolved or the proper authorities are notified.
  • Perform home visits to members to provide health education and ensure continued engagement.
  • Provides telephonic and in-person outreach to address member care gaps and other needs.
  • Thoroughly document and record all interactions with program patients and participants in the community using the data collection procedures established by the department. This may include inputting data and information into spreadsheets, databases, electronic health record systems and others.
  • Writes up work experiences/case stories/anecdotal evidence about program/intervention.
  • Monitor and order project supplies including educational materials and medical devices as needed.
  • Establish, develop, and maintain relationships with community residents, community organizations and stakeholders to further the work of SSHCO in the community and to promote program goals.
  • Assist in conducting surveys such as community health needs assessment and informs leadership of the needs of the service communities.
  • Assist in program-wide recruiting efforts by developing relationships with community-based leaders, organizations and community members and providing approved health education materials and resources to community members and organizations.
  • Collect information from community members and program participants for reports and data to measure program impact. Assist with preparing reports.
  • Represent SSHCO at community events and coalition/community meetings to understand community issues and build relationships with community members.
  • Assist in establishing new SSHCO programs and care models as needed.
  • May present program information at conferences and other professional events.
  • Participate regularly in internal and external professional development opportunities.
  • Perform other related duties and participate in special projects and assignments as assigned.
  • Performs other duties as requested.

QUALIFICATIONS:

  • Flexible work schedule required. The typical schedule is weekdays 8:00am – 4:30pm but shifts may vary, and occasional weekend/evening work may be required.
  • Travel: This position requires regular travel throughout the Chicagoland area. A valid driver's license, reliable vehicle, and proof of insurance are required.
  • Education: High school diploma or the equivalent required.
  • Experience:
    • 2 years of direct patient care or community/patient health experience preferred.
  • Computer Skills:
    • Working knowledge of Microsoft Office Suite (Word, SharePoint, Excel, PowerPoint) preferred.
  • Additional:
    • Familiarity and/or experience working with Chicago's West Side and/or South Side communities (residents preferred).
    • Ability to work in various settings, including home visits, health fairs, community events, clinics, and hospital, emergency room departments or via phone required.
    • Manual dexterity for various duties (e.g., typing, lifting up to 15 lbs.).
    • Ability to provide health education in individual and group settings.
    • Ability to coordinate non-clinical care under supervision.

    • Ensure that actions taken in dealing with students fully conform to established standards.
    • Oversee dorm management and to maximize the benefits of the residential living situation for students.
    • Motivate students and gain their acceptance of and adherence to residential living goals; and an understanding of social behavior and motivation to effectively provide guidance required.
    • Provide individual guidance to students.

What: Subject to management approval of the Regional Administrator, Region Five, Bureau of Community Support Services, Division of Family & Community Services, Illinois Department of Human Services, serves as Team Leader, organizes, plans, and directs staff toward the completion of quality assurance, certifications, and compliance monitoring activities, and assistance with GA TA to bureaus/programs for statewide programs assigned to the work unit. Participates as the Team Leader/Specialist by providing complex inputs into work unit accomplishments and providing technical assistance, training and support to professional staff serving as Community Support Services Consultants (CSSC). Travels statewide in performance of duties, with some overnight.

Job description

The mission of Urban Alliance is to empower under-resourced youth to aspire, work and succeed through paid-internships, formal training, and mentoring. As the primary city-wide internship program in Washington, DC, we accomplish our mission by providing high school students with long-term, paid internships in professional settings where each student's supervisor also serves as his/her mentor. Additionally, we provide job readiness, life-skills, and financial literacy workshops, as well as assistance with college and vocational planning. Over the last 17 years, Urban Alliance has served over 15,000 youth through our High School Internship Program and workshops in the community. Over the last five years, UA youth maintained a 100% high school graduation rate, and facilitated over 90% of program alumni being accepted into college and over 80% enrolling in college. The program has had dynamic growth in the last five years, expanding to serve youth city-wide and in Northern Virginia and opening offices in Baltimore and Chicago.

The Program Coordinator supports the students' mentors and the student interns in their professional development as well as assists them with their post-high school planning. This position reports to the Program Director. We are seeking a candidate who is planning to dedicate a minimum of 2 years to this position.

Job Responsibilities
  • Monitor the professional, academic and personal performance of high school interns
  • Build and maintain relationships with students, mentors, parents, and school staff
  • Actively recruit participants, including visits and presentations to target schools
  • Manage relationships with corporate leaders by supporting mentors with suggestions to best structure intern supervision
  • Conduct formal evaluations of intern job performance and skill development
  • Troubleshoot and mediate jobsite conflicts
  • Facilitate weekly professional development workshops for high school interns
  • Counsel interns on post-high school options, college application process, and develop individual plans for all interns
  • Demonstrate flexibility with the understanding that working with youth requires going above and beyond
  • Work with Executive Director and Director of Evaluation to share data and respond promptly to data requests
  • Represent the organization at events as needed
Qualifications
  • At minimum a Bachelor's Degree
  • At least two year experience working in youth development/ with youth is REQUIRED
  • Curriculum development and teaching experience
  • Is passionate about our mission and the possibilities of what youth can accomplish
  • Experience working with community organizations and schools in under-resourced areas
  • Excellent oral and written communication, including mediation skills and the ability to have direct and open conversations
  • Can work successfully independently and in a team
  • Proficient in MS Word, Excel, Access, Publisher, PowerPoint
