a position opening for a

Clinical Case Manager

For the Opciones Saludables Program


We’re looking for someone who is creative, enthusiastic and has a passion for working with adolescents, particularly with Latina pregnant and parenting adolescents.  The goal of Opciones Saludables is to improve socio-emotional health outcomes among Latino youth and reduce ethnic and racial health disparities. The program focuses on the objectives of increasing knowledge, developing skills and increasing positive behavior among program participants.

This position entails:

·      Providing and coordinating group education based on program curriculum
·      Individual goal-setting and counseling with participants
·      Working with peer-leaders to develop peer led groups and activities
·      Outreach and recruitment of participants
·      Maintaining strong relationships with community partners, including area high schools and internal Wellness & Prevention programs in order to direct participants to needed services


Requirements:
We are requiring a BA/BSW and three years experience or Master’s degree in social work, counseling, psychology or related field. Experience providing services to at-risk youth is desired.

Program Manager

For the Opciones Saludables Program


We’re looking for someone with program development experience and passion for working with the Latino community, particularly with Latina pregnant and parenting adolescents.  The goal of Opciones Saludables is to improve socio-emotional health outcomes among Latino youth and reduce ethnic and racial health disparities. The program focuses on the objectives of increasing knowledge, developing skills and increasing positive behavior among program participants.

The Program Manager will oversee program activities including:

·      Implementation of a new intervention model and curriculum
·      Outreach and recruitment of participants
·      Data management and reporting
·      Supervision of program staff and youth peer leaders
·      Development and maintenance of strong relationships with community partners and internal Wellness & Prevention programs to coordinate program work and referrals for other needed services. 


Requirements:
We are requiring a Master’s degree in Human Services Services related field and at least two years experience providing services to at-risk youth, preferably pregnant and parenting teens. Working knowledge of child development and parenting skills training and at least one year of supervisory experience preferred.
