Youth Housing Care Specialist, Heartland Human Care Services - Hybrid (Chicago, IL)

 Salary Range:

$19.79 To $24.80 Hourly

Title: Youth Housing Care Specialist

Location: 4822 N Broadway Chicago, IL 60640

Schedule: Hybrid, Monday-Friday 8:30am-5:00pm

Summary:  

Under the general guidance of the Program Manager, The Youth Housing Care Specialist will serve as the main point of contact and primary case manager for youth enrolled in the Neon Transitional Housing program. Outcomes are achieved through implementation of participants’ individual service plans (ISP), by providing ongoing housing case management services and connecting youth to needed resources in the community. The Youth Housing Care Specialist leverages industry knowledge and best practices to promote the mission and Vision of Heartland Human Care Services. Supports culture development and management efforts. 

Essential Functions: 

Duties & Responsibilities 

  • Completes intake paperwork with participants and gathers eligibility documentation. Orients participants to the program. Explains paperwork fully and completely. Rapidly engages participants and builds rapport and trust. 
  • Locates and secures appropriate private market housing units for homeless youth and provides advocacy services with landlords on behalf of program participants as needed.
  • Uses Ansell Casey assessment and/or other appropriate assessment tools to create individualized service.
  • Plans, follows up with participants to ensure progress is being made toward identified goals, and provides both case management services and referrals to community resources to assist participants in accomplishing program goals.
  • Utilizes motivational interviewing techniques, maintains clinical understanding of how mental health influences behavior, and uses a trauma-informed approach in working with youth.
  • Monitors the participants’ self-administration of medication as applicable.
  • Completes file reviews and other quality assurance activities as required. Completes and writes case notes daily, uses electronic database(s), completes reports on time, and maintains participant files.
  • Meets with employers, landlords and service providers to help resolve conflicts that might arise.
  • Networks with other housing and youth service providers to ensure that the needs of the youths are
  • adequately addressed.
  • Administers funds for client assistance as per program policies and procedures.
  • Participates in program and operational planning with Program Manager.
  • Continues the professional development through active, self-initiated participation in/with supervision, in house training, conferences, seminars and independent study.
  • Adheres to professional standards as outlined by governmental bodies, NASW (and/or other appropriate professional associations), private funding sources, Organization plans/policies and program guidelines.
  • Initiates requests for assistance from Supervisor to address new issues or complex concepts affecting adherence to professional standards. 
  • Transportation of program participants and/or Organizational property may be required.
  • Works outside of regular work hours with a flex schedule may be required in order to visit participants when they are available.
  • Other duties as assigned.

Senior Housing Services Supportive Specialist, Aids Foundation - Chicago, IL

 

Senior Housing Services Supportive Specialist

Full-Time in  Chicago, IL -  Entry Level -  Housing -  $50k - $52k

The Senior Housing Supportive Services Specialist will coordinate Housing Opportunities for People with AIDS (HOPWA) funded subsidy programs for AFC’s Supportive Housing Programs (SHP). The person in this role will also interface with the Chicago Continuum of Care (CoC) and Coordinated Entry System (CES). This position also serves as AFC’s liaison to housing program sub-contracted partner agencies that provide direct services for chronically medically ill, chronically homeless, and low-income individuals and families. This position also provides guidance and direction to partner agency direct service staff and supervisors about the implementation of the housing program policies and procedures and has shared responsibility for the overall quality of services provided.

The salary range for this position is $50,000 to $52,000 annually.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Program Coordination

• Provide support to case managers and supervisors at various funded partner agencies to provide high quality services

• Provide oversight and direction to supervisors and case managers from partner agencies in accordance with supportive housing program policies and procedures

• Utilize Coordinated Entry System (CES) to track project data and receive project referrals; initiate the referral process to ensure that it is expedited to assess for eligibility, and review and approve eligibility documentation for potential program participants

• Monitor utilization of support services (i.e., case management services, transportation, furnishings, dental, vision); provide information and reports on a regular basis on program outcomes, service coordination, and service delivery to AFC staff and partner agency program directors

• Collaborate with SHP Director & SHP Manager to update SHP policies and procedures and to ensure alignment with current program operations and system-level changes

• Serve as community point of contact and share information for all inquiries about SHP projects

• Collaborate with AFC’s Senior Quality Specialist; provide ongoing technical assistance to partner agency service providers in use of case management database (i.e., Case Worthy)

• Serve as main point of contact for external stakeholders within the Chicagoland Homeless Services System

• Lead short-term projects as needed (i.e., system-level changes to policies and procedures).

Quality Assurance and Data Entry

• Ensure accuracy and timeliness of intake data by entering intakes and exits data on all program participants in data tracking systems (i.e., Case Worthy, & HMIS, and Provide)

• Ensure accurate entry of intake & exit information by partner agencies for accurate reporting

• Communicate with partner agency case managers and supervisors when quality issues arise

• Maintain compliance with all HMIS Data Standards in accordance with HUD CoC requirements by updating referral status when clients are enrolled; complete housing move-in date and exit assessment within 7 days of discharge

• Complete monthly reports for assigned programs

• Participate in annual program partner site visit reviews

• Support SHP Director and Manager in chart audits for Funders

• In conjunction with Program Development & Senior Quality Specialist, monitor data to ensure programs are meeting quarterly and annual Data Quality Standards and the Annual Data Evaluation Standards to ensure programs are maintaining competitiveness with other city-wide CoC Funded Projects

• Field participant calls and complaints as necessary

Meetings and Training Facilitation

• Convene and facilitate System Integration Team (SIT) meetings with partner agency service providers on a monthly basis for multiple housing programs

• Provide guidance or case consultation as need to partner for the development of client-level interventions

• Attend all Department & Inter-departmental meetings (i.e., Payment Processing, Public Grants Prep, SHP Team meeting, Quality Assurance Team meeting & All Housing Team)

• Attend and participate in systems-level meetings and workgroups (i.e., Illinois Department Continuum of Care (CoC), , Coordinated Entry System (CES)

• On-board new partner agencies’ case managers and supervisors on Supportive Housing processes, expectations and policies and procedures

Entry Level Field Economist - HUD (Fort Worth, TX)

Who: US Department of Housing and Urban Development, Office of Policy Development and Research (PD&R), Office of Economic Affairs
PD&R is responsible for providing advice and information to the Secretary to further his policy agenda; for maintaining current information to monitor housing needs, housing market conditions, and the operation of existing programs; and for conducting research on priority housing and community development issues.

The Office of Economic Affairs, Economic and Market Analysis Division, within the Office of the Assistant Secretary for Policy Development and Research, is responsible for collecting data, conducting economic research, developing economic models, and performing evaluation and policy analysis in the following areas: economic, fiscal, and social conditions in cities and other communities; strategies to promote economic development in cities and other communities; the impact of taxes and tax changes on housing markets and urban conditions; and the low income housing tax credit.

Housing Specialist, Garfield Park Community Council - Chicago, IL

Background:

The mission of the Garfield Park Community Council (GPCC) is to strengthen, promote and encourage opportunities that support an engaged, equitable and vital Garfield Park neighborhood. Spun off by the Garfield Park Conservatory Alliance in 2012, the GPCC is building on its work of the past nine years to revitalize and bring new opportunities to Garfield Park. The GPCC’s housing activities aim to create and maintain affordable, quality housing options for both longtime residents and attract new families to the neighborhood.
Between 2009 and 2011, over 20% of all Garfield Park occupied rental properties were impacted by foreclosure. In response, the GPCC created the Garfield Park Housing Center, which provides free housing counseling and referrals for local renters and landlords. In 2013, the GPCC partnered with the City of Chicago to implement the Micro- Market Recovery Program (MMRP), a new initiative to kick-start the residential housing market in communities affected by high rates of foreclosure. The GPCC leads MMRP efforts in East Garfield Park, targeting specific blocks for assistance. In 2015, the GPCC will integrate the MMRP and the Housing Center services with the GPCC’s broader community development efforts, which will include organizing and training resident advocates and developing leaders.
The GPCC seeks to hire a housing specialist to provide housing counseling and referrals for renters and landlords and to work with the City of Chicago and MMRP partners to support existing homeowners to maintain their properties and partner with developers and homebuyers to reoccupy vacant properties. The Housing Specialist will report to the GPCC’s Executive Director.
Responsibilities:

  •   Research and assist with securing additional grant funding consistent with housing program objectives
Qualifications:
Candidate should have Bachelor’s Degree (minimum) in related field. Experience in urban, community development and organizing required and demonstrated ability in housing/real estate preferred; excellent interpersonal skills; and strong oral and written communication skills. The person must be willing to work evenings and weekends. Real estate professional background a plus. The ideal candidate will be familiar with Chicago’s Resident Landlord Tenant Ordinance.
