Population Health Navigator, Erie Humboldt Park Health and Dental Center - Hybrid (Chicago, IL)

The Population Health Outreach Program supports patients by phone and works mainly remotely with clinical care team members across all sites. As part of the role, the Population Health Navigator role is a patient-facing role that helps improve the agency’s quality of health care through targeted, data driven outreach. Outreach is based on
agency priorities, including chronic disease and preventive health needs and specialty care referral navigation, and includes a variety of methods such as campaign texting, 1on1 patient phone calls, bulk referral ordering, MyChart, as well as others. Maximus is looking for YOU!  We are looking for an Outreach Coordinator working with our LA paternity project.

The Outreach Coordinator conducts in-person trainings with hospital and birthing center staff across the state on the importance of having both parents documented on a child’s birth certificate. He/she also builds relationships with local community partner organizations to help inform expectant families on the birth documentation process.

** Remote (LA) position with 20% Travel within the state of Louisiana**

Essential Duties and Responsibilities:

- Conduct regular one-on-one or group training sessions for existing providers, necessitating travel.

- Attend and actively participate in recruitment events to attract new providers to the program.

- Handle escalated provider issues efficiently and effectively.

- Address urgent concerns related to high visibility providers, ensuring swift resolutions.

- Offer ongoing support to designated providers as assigned.

- Collaborate with client, fellow contractors, and stakeholders to communicate and implement - operational policy changes.

- Stay updated on project details and changes.

- Extensive travel is an inherent part of the role.

- Fulfill additional assignments as directed by management.

Outreach Specialist, Wounded Warrior Project - Hybrid (Chicago, IL)

 DUTIES & RESPONSIBILITIES

  • Connect with wounded service members, their families, and family support members, who are in transition or have separated from military service. Establish rapport with them and communicate the benefits of WWP programs and services provided for wounded warriors.
  • Recruit wounded warriors, family support members and other support members to participate in WWP events and activities.
  • Plan local or regional events and activities for the benefit of Alumni and their families and encourage them to participate and connect with other Alumni.
  • Develop and facilitate virtual programming content via Zoom, Facebook Live, and other technology-based platforms.
  • Coordinate details for and execute these Alumni events and activities, providing networking, camaraderie-building, and family bonding opportunities.
  • Assess the needs of Alumni and connect them with WWP programs and services that will assist with their long-term recovery and empowerment.
  • Identify community events and activities in the local and regional area that will provide opportunities for warrior participation and connection.
  • Regularly communicate and network with wounded warriors, serving as a resource and mentor.
  • Establish effective working relationships and work collaboratively with WWP teammates.
  • Utilize expense controls and seek cost-saving opportunities to help manage budget effectively.
  • Support WWP by serving as local representative at events. Serve as point-of-contact for WWP Alumni and their family members and ref them to appropriate WWP programs, resources, or staff.
  • Develop community support for WWP. Present WWP vision and values to the community, communicating the benefits of WWP programs and services, and inspire them to become involved with and support the WWP vision.
  • Maintain current and accurate data in WWP intranet electronic tracking systems, e.g., SalesForce. Collect and report results and performance metrics for Alumni events and activities.
  • Develop and build relationships with military-focused nonprofit organizations and community groups. Locate wounded warriors who have not had contact with WWP and introduce WWP’s programs and services.
  • Serve as local WWP representative when collocated near Medical Treatment Facilities or VA poly-trauma units and military facilities where wounded service members receive treatment.  Meet warriors at bedside and work directly with them and their family members to assist their recovery and transition process.
  • Other related duties as assigned.

Outreach Associate - North Branch Works - Chicago

 Outreach Associate (Part-Time)

• The Outreach Associate is an important role in North Branch Works’ new nonprofit
capacity building program that was recently funded by the City of Chicago.

• The primary focus of this position is to connect with potential nonprofit partners in the following
neighborhoods: Avondale, East Garfield Park, Hermosa, Humboldt Park, Irving Park, Near West
Side, Garfield Park, and West Town.

• The Outreach Associate will lead the initial contact with these organizations and help strengthen
NBW’s network in West Side communities. This task will require research, virtual and in-person
meetings, phone calls, direct mail, and helping to provide technical assistance. The Outreach
Associate will build upon our asset map for West Side neighborhoods and further strengthen our
directory.

• The Outreach Associate will work closely with our Director of Business and Economic
Development to help nonprofit organizations identify and apply for grant opportunities that build
their capacity to deliver high-quality services and carry out their missions.

• The Outreach Associate will help NBW organize and deliver two webinars targeted to nonprofit
organizations in Q4 of 2022 and Q1 of 2023. This work may include opportunities for public
speaking, program development, event planning, and group facilitation.

• The Outreach Associate will participate in community roundtables and other meetings to identify
and partner with potential nonprofit partners.

• The position also requires updating records in Salesforce and adding new nonprofit partners.

• The Outreach Associate may participate in meetings with businesses and decision-makers and
help with our 2023 events such as Chicago River Day Cleanup and North Branch Brewery Tour.

• The Outreach Associate will help with communications work such as social media and writing
stories for our blog and our bi-weekly e-newsletter.

• This position is part-time (20 hours) and contingent on grant-funding through September of 2023.

• Bilingual English/Spanish speaking candidates are preferred.

• Compensation for the Outreach Associate position is $18 per hour.

District Communications Coordinator & Outreach Liaison, Congressman Jesús "Chuy" García

 Job Description: District Communications Coordinator & Outreach Liaison 

The District Communications Coordinator and Outreach Liaison is essential to the overall communications efforts of the congressional office. This position is responsible for effectively maximizing communication about the Congressman’s accomplishments, priorities, and events to constituents, stakeholders, and local press throughout the District. This position requires excellence in communications, digital platforms activity, as well as community engagement. This position will also carry out duties to implement the district outreach strategy, community relations, and constituent services. This position reports to the District Director and works closely with the Communications Team and Outreach Manager for communications and outreach, respectively. 

QUALIFICATIONS: 

Excellent writing and editing skills in English and Spanish; 

Strong knowledge of organizations, institutions, and leaders in the 4th congressional district.

Knowledge of 4th congressional district history and issues; and citywide issues. 

Knowledge of local and citywide reporters, bloggers, and media outlets. 

Ability to plan and edit visual material to support communications, including photos, video and graphics;

Understand and implement best practices and most effective use of different communications and social media platforms; 

Understand how to use communications as a tool for community organizing and outreach among a wide variety of district stakeholders; 

RESPONSIBILITIES: 

Act as a liaison to stakeholders, coalitions, constituents, and federal government agencies to develop and maintain effective relations; 

Provide input to communications calendar and implement District based communications projects; 

Work with the Communications Director to develop and maintain relationships with Chicago-based reporters, bloggers, communicators. Be able to schedule and staff interviews for member; 

Develop and maintain District communications contact list, including radio, TV, neighborhood news, bloggers, local news outlets. Coordinate with the DC Communications team; 

Develop a list of social media “influencers” in the district to follow and to invite as amplifiers of the member’s work; 

Work with District and DC staff to send targeted emails about Congressman’s work to groups of influencers and stakeholders in the district; 

Work in coordination with Outreach Manager to identify key constituent stories to highlight and share with DC Communications team; 

Help District Director plan events that have excellent visual and social media elements

Follow events and leaders in the District to regularly generate local and “personal touch” stories to highlight via Member communications; 

Coordinate local press engagement opportunities and events like press conferences, gaggles, etc.;

Assist with planning and communications for major district events like town halls; 

Assist with planning and communications for rapid response projects; 

Assist with other communications, outreach and event projects as assigned; 

Work collaboratively with Outreach Manager and team to implement district-wide outreach plan based on assigned outreach areas 

Inform the District Director and Outreach Manager of timely and relevant updates of outreach areas; follow areas at the federal, state, and local levels. 

Meet regularly with stakeholders to gather insight and recommendations on policy impact; relay to District Director and Outreach Manager;

Prepare periodic reports for communications, outreach, and casework efforts as established by respective teams 

Coordinate district events such as roundtables, townhalls, meetings, tours, corridor walks, etc. as needed with team; 

Maintain up-to-date files and status on all cases and outreach efforts; 

Work collaboratively with Constituency Services Manager on assigned casework based on respective areas;

Meet attendance requirements as established by the office; 

Maintain a good working relationship with the Member, staff, and constituents; 

Perform other duties as assigned 

EDUCATION/EXPERIENCE: 

At least two years of community organizing, campaign, advocacy, communications or government experience. A post-secondary degree is preferred. Spanish-language fluency or proficiency is preferred.
