SNAP Case Manager - TCA Health Inc.

Who: TCA Health Inc.

What: SNAP Case Manager Works closely with the program Coordinator and clients who have been referred by DHS to participate in the Employment & Training Program (E&T).


Duties & Responsibilities:
  • Assist with monthly placement assignments of eligible participants.
  • Work with the Program Coordinator to ensure that participants are properly matched in placement sites.
  • Meet one on one, at office, with new referrals to verify eligibility
  • Meet one on one, with program participants to assess specific needs, identify goals, and prepare action plans for achieving the goals.
  • Support clients in development and updating resume’s, completing applications, coaching on employment soft skills, and removing barriers to employment by linking to community resources
  • Keep and maintain accurate and up to date client files at the office. This includes entering case notes and data in DHS database and client’s file.
  • Track and report outcomes and provide data updates on participants as directed.
  • Participate in on site and off site trainings and meetings, as needed.
  • Assist Program Coordinator in prepare for monthly staffing at local FCRC offices
  • Attend training and staffing’s as needed and prepare for program reviews
  • Actively participate in the quality and risk management activities of the organization
  • Other duties as assigned.
Other Skills and Abilities:
  • Experience with Employment and training programs and participants, preferred.
  • Excellent Case Management skills.
  • Interviewing skills and knowledge of employment guidelines a plus.
  • Demonstrates written and oral communication skills, ability to works as part of a team
  • Demonstrates strong skills in interviewing, engagement, assessment, planning and advocacy
  • Demonstrates good judgment and ability to work independently
  • Interviewing skills and knowledge of employment guidelines.
  • Knowledge of standard business practice and management.
  • Basic Computer skills, Microsoft word and Excel
Education Requirements
  • Bachelor’s Degree in related field of social services
  • Human resources or business equivalent with minimum one years of experience in working with workforce development in a for profit setting
